Inclement Weather Closing

In the event of poor/inclement weather or other disaster, please call the office and the recorded message will advise of any closings or late openings.

Cancellation Policy

You must provide 24-hours notice to cancel your appointment. Monday appointments must be cancelled on Friday. Failure to adhere to this policy will result in a $45 late fee.

Payment & Sliding Scale

Claims to your individual insurance companies will be submitted on your behalf. Individuals will not be denied access to services based on inability to pay & a discounted/sliding scale is available. Co-payments, deductibles and self-payments must be paid at each visit. Cash, check, and credit cards are accepted. Contact us for more information.
